Stay Connected to Your Patients — Even When They’re at Home
Remote Patient Monitoring (RPM) from Axial Patient Care empowers you to monitor vital health data daily, make timely clinical decisions, and earn additional revenue — all without adding strain to your practice.

Why Remote Patient Monitoring Matters
Chronic Conditions Don’t Pause Between Office Visits
Patients managing hypertension, diabetes, heart disease, and other chronic conditions often experience health changes long before their next scheduled appointment.
Without access to real-time data, critical signs of worsening health can be missed — leading to emergency visits, hospitalizations, and lost opportunities for early intervention.
Remote Patient Monitoring (RPM) bridges that gap — giving you daily insights into your patients’ conditions and the ability to act sooner.
How Our Remote Patient Monitoring Program Works
Simple for Your Patients. Powerful for Your Practice.
- Device Deployment: Axial provides patients with easy-to-use monitoring devices such as blood pressure monitors, glucometers, or other tools tailored to their health needs.
- Daily Data Transmission: Patients use their devices from the comfort of home. Readings are automatically transmitted to our clinical team.
- Alert Monitoring: Any out-of-range values generate immediate alerts. Our nurses reach out to the patient, assess symptoms or contributing factors, and escalate to your office if needed.
- Clinical Reporting: We provide clear, actionable reports to support your care decisions — while maintaining thorough, compliant documentation for billing.
You receive timely insights — without additional workload on your staff.

Remote Patient Monitoring (RPM) – FAQ
1. Which patients are good candidates for RPM?
RPM is ideal for patients with hypertension, diabetes, heart failure, obesity, or other chronic conditions that require regular monitoring of vital signs like blood pressure or glucose.
2. What devices are used for RPM?
Axial provides FDA-cleared Bluetooth-enabled devices such as blood pressure monitors and glucometers. Data is transmitted automatically to our clinical dashboard for daily review.
3. Who reviews the patient data and responds to alerts?
Our clinical team reviews all incoming data daily. If readings are outside of physician-defined parameters, a nurse contacts the patient, assesses the situation, and notifies your office when clinical action is needed.
4. Is RPM reimbursed by Medicare?
Yes. RPM is reimbursed using CPT Codes 99453, 99454, 99457, and 99458. Reimbursement can total $120–$160+ per patient per month, depending on engagement and time spent.
5. Will this add work to my staff’s day?
No. Axial Patient Care acts as an extension of your team. We handle device setup, patient support, daily data review, alert triage, and documentation — keeping your workflow efficient and compliant.
